Surgery for Inferior Oblique Overaction

ABSTRACT The inferior oblique weakening operation is the most common performed vertical muscle procedure. The surgical procedure usually selected to weaken clinically overacting inferior oblique muscle are tenotomy, disinsertion, recession, myectomy, denervation and extirpation. Each of which has advantages and disadvantages. Opinions continue to vary regarding its optimal technique.

A review of published reports revealed that each technique appeared to produce the intended result but that investigation differed in their choices for reasons of simplicity, predictability, quickness and complications. Complication of the inferior oblique weakening procedures are residual overaction, underaction, adherence syndrome, overaction of contralateral inferior oblique and internal ophthalmoplegia.
