ABSTRACT We report a 70 year-old male patient who developed anterior ischemic optic neuropathy (AION) 13 days after phacoemulsification with intraocular lens implantation Fundus examination revealed pale swollen disc with splinter hemorrhage. Fundus fluorescein angiography showed a sector of peripapillary choroidal filling defect. Post-cataract-extraction-AION is a rare but serious postoperative complication which ophthalmologists should be aware of. Fellow eye appears at substantial risk of similarly developing AION following cataract surgery
