After iridotomy,
Southeast Asian patients may benefit from SLT treatment
This study assessed the efficacy of selective laser trabeculoplasty (SLT) in Southeast Asian eyes with primary open-angle glaucoma (POAG) or primary angle-closure glaucoma after peripheral iridotomy (PACG-PI). Sixty-three POAG and 12 PACG-PI eyes were included; the mean patient age ranged from 60.3 to 62.9 years. In the POAG group, the mean IOP was 19.4 mm Hg at baseline and 14.1 mm Hg at 24 months post-SLT; in the PACG-PI group, these numbers were 20.7 mm Hg and 13.6 mm Hg, respectively. Results indicate that in the highly pigmented eyes of Southeast Asian populations, SLT may be an option to further lower IOP in eyes with angle closure with visible trabecular meshwork after iridotomy. Patients with systemic sclerosis are at risk of developing
dry eye disease
A prospective cross-sectional study of 84 patients with systemic sclerosis (SSc) was conducted to evaluate the prevalence of dry eye disease (DED) in this patient population and assess the predictive factors associated with DED occurrence. The Ocular Surface Disease Index (OSDI) was used to assess the symptoms of DED; objective tests included tear breakup time and ocular surface staining. Diagnosis of DED was made with the DEW II method. Fifty-two percent of the patients with SSc were found to have DED, and DED occurrence was associated with older age and longer SSc duration. Therefore, the authors recommend administering the OSDI and objective tests for DED to all patients with SSc. Clinical Ophthalmology, October 2022

Fractionated stereotactic radiotherapy for meningioma has
positive long-term effects
A 22-year retrospective study of 34 patients with primary optic nerve sheath meningioma who were treated with fractionated stereotactic radiotherapy evaluated long-term visual function (VF), including visual acuity and visual field mean deviation, and complications. Approximately 90% of patients exhibited long-term treatment success in terms of both overall VF and individual VF measures. The tumor control rate, defined as stable or reduced tumor at the final magnetic resonance imaging follow-up, was 100%. Only one patient had radiation retinopathy after treatment. Improved visual
outcomes seen with immunosuppressive agents for isolated retinal vasculitis
Investigators conducted a retrospective chart review of 48 patients (73 eyes) with isolated retinal vasculitis to determine prognostic factors for poor visual outcomes. Following confirmation of the active retinal vasculitis diagnosis by fluorescein angiography, nearly all patients were treated with either immunosuppressants or systemic corticosteroids. Improvements in best-corrected visual acuity (BCVA) were noted at the 6- and 12-month follow-up visits. Ten percent of patients had poor visual outcomes, which were associated with lower initial BCVA and the presence of outer retinal disruption. Retina, October 2022

Co-infection with EBV
and Talaromyces marneffei is seen in a case of sclerouveitis
This report focuses on a rare case of Epstein-Barr virus (EBV) and Talaromyces marneffei co-infection in a 47-year-old HIV-positive woman who presented with infectious sclerouveitis and a vascularized iris tumor in the left eye. After the ancillary laboratory workup, EBV was detected by polymerase chain reaction on aqueous humor and scleral tissue analysis. The lesion subsided completely with systemic and local ganciclovir therapy, but a recurrent scleral nodule with iris plaque subsequently developed and T. marneffei infection was diagnosed pathologically. Treatment with both intravenous and intracameral amphotericin B led to a significant regression of the lesion. Annals of Medicine and Surgery (London), August 2022

Eviscerations or enucleations
may be performed by supervised ophthalmology residents
The
surgical outcome and complications of evisceration or enucleation procedures
with primary implantation that were performed by ophthalmology resident trainees
were evaluated in this study. Sixty-six patients with recalcitrant
endophthalmitis or panophthalmitis were included. Four patients had implant
exposure and 4 had implant extrusion; a multivariate analysis demonstrated that
the presence of Pseudomonas aeruginosa infection and not receiving Intravitreal
antimicrobial drugs before the eye removal procedure were associated with
implant exposure or extrusion. Similar rates of surgical complications and
successful prosthesis fittings were noted between the resident trainees and
experienced staff. Therefore, these 2 procedures could potentially be performed

Prompt treatment with antibiotics can improve visual
outcomes in syphilitic uveitis
Data from 36 patients with syphilitic
uveitis treated at a single hospital were used to identify the clinical features,
longitudinal pattern, and rate of visual acuity (VA) improvement following
antibiotic therapy. Most patients (81%) had HIV co-infection, and 49% presented
with panuveitis. The mean VA improved from 0.97 at baseline to 0.39 logMAR at 3
months and remained stable thereafter. Seventy percent of patients achieved VA
z20/25 by 2 years post-treatment. Receiving antibiotic therapy within 4 weeks
of the onset of ocular symptoms, absence of HIV co-infection and neurosyphilis,
better presenting VA, and having intermediate uveitis rather than panuveitis

Choroidal vessel
density is reduced to a lesser extent in acute vs chronic glaucoma
In this prospective cross-sectional study, investigators evaluated whether parapapillary choroidal microvasculature (PPCMv) density, as measured by OCT angiography, differed between patients with acute primary angle-closure (APAC), patients with primary open-angle glaucoma (POAG), and controls. Data from 40 eyes with APAC, 65 eyes with POAG, and 44 healthy eyes were analyzed. Both groups with glaucoma had significantly lower PPCMv density compared with the control group; however, mean outer annular PPCMv density was 3.6% lower in patients with POAG than in patients with APAC. The mean differences in inner and outer superior PPCMv between the POAG and APAC groups was 5.9% and 4.4%, respectively. British Journal of Ophthalmology, in press

Intravitreal dexamethasone implants are effective for treating
secondary macular edema
A real-world, observational study
evaluated the efficacy and safety of intravitreal dexamethasone implants
(Ozurdex) in 44 eyes with macular edema associated with retinal vascular
disease. Significant improvements in both logMAR best- corrected visual acuity
(BCVA) and central retinal thickness (CRT) were noted over 6 months, though
patients with thicker CRT at baseline were more likely to need additional
injections. The mean logMAR BCVA was greater in patients with retinal vein
occlusion than in patients with diabetic macular edema, and in treatment-naïve
eyes vs previously treated eyes. Nine eyes had increasing IOP, but this was
well controlled with the implants; cataract progression was observed in 5 eyes

A smartphone app provides important data on symptomatic dry
eye in Thailand
To assess the prevalence of and risk factors for symptomatic dry eye (DE) in Thailand, investigators reviewed data from the "Dry Eye or Not?" smartphone app, developed by the Cornea and Refractive Surgery Society of Thailand, on ocular surface disease index (OSDI) score, blink rate, maximum blink interval (MBI), best spectacle-corrected visual acuity (BSCVA), and visual display terminal (VDT) use. The criterion for symptomatic DE was an OSDI score 213. The prevalence of symptomatic DE among 9482 individuals using this smartphone app was 85.8%. Female sex, worse BSCVA, increased blink rate, reduced MBI, and >6 hours of daily VDT use were significantly associated with symptomatic DE. Screening for symptomatic DE and evaluating the signs and risk factors for DE via this app may allow for the development of strategic plans for health care systems in Thailand.
