Fewer failures seen when
phacoemulsification is added to trabeculectomy
A retrospective review focused on 121 phakic
patients (146 eyes) with primary glaucoma who underwent either mitomycin C-augmented
trabeculectomy with phacoemulsification (Phaco + Trab) or trabeculectomy alone
(Trab) and completed 2 years of follow-up. The Phaco + Trab group had a
significantly lower failure rate than the Trab group, but differences in postoperative
IOP, percentage of IOP reduction, and number of glaucoma
medications were not significant
between the groups. The impact of adding phacoemulsification to trabeculectomy
was found to be similar in eyes with open-angle glaucoma and eyes with angle-closure
glaucoma.

In Thailand, retinal vasculitis is most commonly caused by
Behcet disease
This retrospective cohort study
analyzed the characteristics, etiology, and outcome of retinal vasculitis in
central Thailand. Ten percent of 741 patients with uveitis were found to have
retinal vasculitis. The most common cause of retinal vasculitis and vascular
leakage on angiography was Behcet disease. Seventeen percent of retinal vasculitis
cases eventually required vitrectomy; 43% Of cases complicated by ischemia
required photocoagulation and
anti-VEGF injections.

Both surgery and botulinum toxin injection have advantages
in treating esotropia
The success rates of extraocular
muscle surgery and botulinum toxin injection in 114 patients with acute
acquired comitant esotropia (AACE) were compared in a
retrospective cohort study. Angle of deviation
in prism diopter was measured at baseline and at 1, 3, and 6 months after treatment.
Mean reductions in esodeviation angles at all timepoints after treatment were
significantly greater in the surgery group than in the botulinum toxin group; surgical
success rates were also significantly higher in patients given surgery than patients
given botulinum toxin (79% vs 27%, respectively). While botulinum toxin is not
as effective as extraocular muscle surgery in patients with AACE, because it is
safe, fast-acting, and improves both cosmetic
appearance and quality of life it may be considered for patients who prefer to
not have surgery.

Bilateral PION is a rare but severe complication of carotid
cavernous fistulas
The authors report a case of bilateral
posterior ischemic optic neuropathy (PION) due to bilateral anterior-drainage
dural carotid cavernous fistulas (CCFs) in a 62-year-old woman with a history
of poorly controlled hypertension who presented with sudden bilateral visual
loss and headache for 5 days. On examination, her visual acuities were no light
perception (NLP) in both eyes. Anterior segment examination revealed bilateral
chemosis and episcleral
corkscrew vessels, and fundus
examination showed slightly dilated and tortuous retinal
veins with normal-appearing optic
discs in both eyes. Diffusion restriction on diffusion-
weighted imaging in the entire
bilateral orbital segment of the optic nerves was revealed, consistent with
bilateral PION. While there was progressive improvement of ophthalmic signs 3
months after transvenous coil embolization treatment, the patient's visual
acuities remained NLP in both eyes.

Removing silicone oil after retinal reattachment does not
always improve outcomes
A retrospective cohort study evaluated
the predictors for visual outcome after silicone
oil removal (SOR) in 182 eyes with complicated
retinal detachment. The results showed that at 3 months following SOR, 91% of
eyes had anatomical retinal reattachment; 57% of these eyes achieved good
visual outcome (BCVA 220/200). However, in the eyes that remained
attached after SOR, 63% achieved good visual
outcome. Univariate and multivariate
analyses found that predictors for
good visual outcome included better BCVA before
SOR and pseudophakia status at the
last follow-up visit.

Preoperative_pRNFL thickness may predict
visual function following adenoma surgery.
Researchers evaluated the prognostic
ability of preoperative peripapillary retinal nerve
fiber layer (pRNFL) thickness for
predicting postoperative visual functions, including the
visual field index (VFI) and visual
acuity (VA), in 33 patients (66 eyes) with pituitary
adenoma who underwent endoscopic transsphenoidal
surgery. The results showed
that thicker preoperative temporal and
inferior pRNFLs were associated with a postoperative VFI >90%, and a thicker
preoperative inferior-quadrant pRNFL was
associated with a postoperative VA of
at least 20/25.

Diabetic retinopathy
and poor glycemic control affect the ocular surface microbiome
Next-generation
sequencing of conjunctival swabs from 60 eyes of patients with diabetes and 20
eyes of patients without diabetes was undertaken to assess differences in
ocular surface microbiomes between the 2 populations. Patients with diabetes
were further classified by diabetic retinopathy (DR) stage and by glycemic control.
Overall, the diabetes group had a higher culture-positive rate than the group without
diabetes. Pathogenic organisms and antibiotic-resistant strains were more common
in those with DR, especially the Enterobacteriaceae and Neisseriaceae families
and the Escherichia-Shigella and Pseudomonas genera. The microbial communities
in the poorly controlled diabetes group were also significantly different from
the well-controlled diabetes group and the group without diabetes.

Improvements in IOP
are seen in eyes with ciliochoroidal effusion treated with laser therapy
In
a prospective study, 50 glaucomatous eyes that underwent micropulse
transscleral laser therapy were assessed for ciliochoroidal effusion by
anterior segment OCT at 1, 4, and 12 weeks following surgery. Eyes were grouped
into those with or without early postoperative ciliochoroidal effusion. At 1
week, 46% of eyes had developed ciliochoroidal effusion, but almost all
effusion cases resolved at 4 weeks. Eyes with ciliochoroidal effusion had
significantly greater IOP reduction and lower absolute IOP at the 4-week visit
than eyes with no effusion. There was no significant difference between the
groups in the number of
medications
needed.

Curcuminoids have_potential in improving vision in_patients
with G11778A LHON
This randomized, double-blind,
placebo-controlled trial assessed the potential role of
curcuminoids (an antioxidant) in
vision and blood oxidative status in patients with
G11778A Leber hereditary optic
neuropathy (LHON). Forty-nine patients (97 eyes) were
randomly assigned to receive either
oral curcuminoids capsules (250 mg twice daily)
or placebo for 48 weeks. Visual
parameters, markers for oxidative stress, and endogenous antioxidants in
erythrocytes were measured at all timepoints. The results showed curcuminoids
at this dose did not have significant effects on the visual and blood oxidative
status compared with placebo. However, slight improvements in Humphrey visual
field mean deviation and a trend of reduction of glutathione peroxidase in the
curcuminoids group suggest that further studies with higher doses of curcuminoids,
particularly in patients at the acute stage of the disease, could be considered.

In Thai patients with
LHON, G11778A is the most common genetic mutation
Leber's
hereditary optic neuropathy (LHON),the most common mitochondrial optic neuropathy,
causes visual loss in young adults, but epidemiologic data in Southeast
Asian
populations are limited. This study reports demographic data, mutation types,
and
visual prognosis in 229 Thai patients with LHON, 210 of whom were young adult
men.
The most common mutation was the G11778A type, followed by the T14484C type.
Patients with the G11778A mutation had a younger age of LHON onset than
those
with the T14484C mutation (21.9 vs 33.0 years, respectively). Presence of the G11778A
mutation, male sex, and older age were linked to worse visual acuity and poorer
prognosis.

Panophthalmitis and endophthalmitis increase the risk of
globe removal after OGI
Investigators conducted a
retrospective chart review to evaluate the prevalence of and risk factors for
globe removal among 422 patients in agricultural regions of southern Thailand
who had open-globe injuries (OGIs). The highest prevalence of OGIs was observed
in those aged >20 to 40 years. The most common cause of OGI was
agriculture-related injury (55%), followed by industry- or workplace-related
injury, assaults, and recreation-related injury. Globe removals were performed
in 12% of eyes with OGIs; the most commo
n risk factors for globe removal were

Vitamin D deficiency. is common in patients with
immune-based optic neuritis
In this case-control study, serum
vitamin D levels and status were measured in 59 patients with immune-based
optic neuritis (ON) and 236 age- and sex-matched healthy controls. Mean serum
total vitamin D levels were significantly lower in the patients with
immune-based ON than in the healthy controls, with all patients with ON having
vitamin D insufficiency or vitamin D deficiency. However, serum vitamin D
levels were not associated with ON attack severity. Screening for serum vitamin
D levels is highly recommended in all patients with acute immune-based ON,
regardless of the specific ON etiology.
