Automated segmentation prior to OCT classification improves classification accuracy
Investigators compared the performance of automated segmentation prior to classification of OCT images of macular diseases vs automated classification without segmentation. Images were collected from eyes with macular diseases (e.g.,neovascular AMD, diabetic macular edema) as well as the normal fellow eyes. A total of 14,327 OCT images were used to train the deep-learning (DL) models. The sensitivity, specificity, and accuracy of classification alone were 62.6%, 95.2%, and 93.1%, respectively, whereas the values of those factors for segmentation plus classification were 71.4%, 96.4%, and 94.8%, respectively. Pigmentation on central anterior lens capsule is one risk
factor for HLA-B27-AU
Data from 268 patients (320 eyes) with anterior uveitis (AU) were used to develop 2 diagnostic models to predict the probability of human leukocyte antigen B27-associated AU (HLA-B27-AU). The first model included 6 variables: age, gender, unilaterality, presence of nongranulomatous keratic precipitates, hypopyon, and IOP. The second model added the value of pigmentation on central anterior lens capsule (PioLe) into the first model. Unilaterality, hypopyon, IOP<21 mm Hg, and PioLe were found to be significantly correlated with the presence of HLA-B27-AU. Ocular Immunology and Inflammation, in press 2023

Direct and dural carotid cavernous sinus fistulas have
distinct characteristics
Investigators compared the clinical characteristics of direct and dural carotid cavernous sinus fistulas (CCFs) in 60 patients, and found that patients with direct CCFs were more likely to be male, of younger age, have a history of trauma, and have greater visual impairment at presentation than patients with dural CCFs. In addition, those with direct CCFs had significantly more chemosis, proptosis, orbital bruit, and dilated retinal vessels than those with dural CCFs. Half of all patients had increased IOP, with significantly higher IOP seen in the affected eyes than in the unaffected eyes. Information on these clinical characteristics may be helpful in distinguishing direct CCFs from dural CCFs. Clinical Ophthalmology, April 2023

Central-hole implantable collamer lenses improve vision
with few side effects
This systematic literature review
focused on the efficacy, safety, and patient outcomes of
phakic IOL implantation using an implantable
collamer lens (ICL), particularly newer models with a central hole. Data from
studies on hole-ICL implantation published between October 2018 and October
2022 (3399 total eyes) indicate improved vision and quality of life with
these ICLs, with a low incidence of complications
such as cataracts, elevated IOP, or corneal endothelial cell loss. The authors
conclude that ICL implantation appears to be a promising refractive surgical

Many older patients with visual impairment experience
suicidal ideation
Researchers evaluated the prevalence
of suicidal ideation and associated factors among 314 patients aged 260 years
with visual impairments who attended an eye center during the COVID-19
pandemic. Suicidal ideation was measured using the Columbia-Suicide Severity
Rating Scale. The prevalence rate for suicidal ideation was 32.5%; risk factors
included the presence of depression (OR 6.1), kidney disease (OR 3.0), diabetic
retinopathy (OR 1.8), and
hypertension (OR 1.7). Having both depression
and diabetic retinopathy increased the likelihood of suicidal ideation sevenfold.

Botulinum toxin type A may improve blepharospasm outcomes
and patient HROOL
A prospective, multicenter,
observational study was conducted to compare health-related quality of life
(HRQOL) and severity of disease before and after treatment with botulinum toxin
type A (BTX-A) in 184 patients with daily-life-affected benign essential
blepharospasm (BEB). Quality of life was evaluated by the Thai versions of 2 HRQOL
questionnaires, and severity and frequency of BEB was evaluated by the Jankovic
rating scale. The results showed that after BTX-A treatment, HRQOL improved
significantly in most of the dimensions of the questionnaires, and severity of

Intraocular sporotrichosis can masquerade as bilateral
granulomatous panuveitis
Investigators describe the case of a
62-year-old woman presenting with blurred vision in both eyes, generalized erythematous
papules, and bilateral granulomatous panuveitis. Cultures identified the
presence of Sporothrix schenckii, leading to a diagnosis of intraocular
sporotrichosis secondary to disseminated sporotrichosis. The patient was
treated with intravenous liposomal amphotericin B and intravitreal amphotericin
B, with subsequent resolution of the skin lesions and intraocular inflammation.

Patients with low vision reported increased depression
during the pandemic
A cross-sectional study examined the association between the impact of the COVID-19 pandemic and depressive symptoms in 289 adult patients with low vision or blindness. Face-to-face interviews were conducted, and measurements included the Multi-Dimensional Scale of Perceived Social Support, the Impact of COVID-19 Questionnaire, and The Center for Epidemiological Studies-Depression questionnaire. Forty-three percent of patients reported increased depressive symptoms during the pandemic, including tiredness and loss of interest or pleasure. Compliance with COVID-19 prevention strategies and presence of anxiety had positive associations with depressive symptoms, while social support had a negative association with depressive symptoms. Long-term improvement is seen in children with uveitis and
severe visual impairment
Investigators studied 5-year visual outcomes
of 51 children (57 eyes) with uveitis and severe visual impairment (<20/200)
at presentation. Seventy-five percent of patients had unilateral uveitis; the
most common causes of uveitis were ocular toxoplasmosis and toxocariasis. Overall,
mean visual acuity significantly improved during the study period, from 1.8 logMAR
at presentation to 1.2 logMAR at 5 years. Risk factors for poorer visual outcome
included younger age at uveitis onset, delayed presentation (21 month), and

Antibody titers can identify complications in specific
ocular myasthenia gravis cases
An 11-year retrospective study evaluated the association between acetylcholine receptor (AChR) antibody titers and conversion to generalized myasthenia gravis (GMG), the presence of thyroid autoimmune antibodies, and the presence of thymoma in 118 patients with AChR antibody-positive ocular myasthenia gravis (OMG). At the final follow-up visit, 16% of patients had converted to GMG; there was a strong association between an AChR antibody titer 28.11 nmol/L and GMG conversion (OR 3.66). An AChR antibody titer 22.81 nmol/L was associated with thyroid autoimmune antibodies, while an antibody titer 215.12 nmol/L was associated with thymoma. Therefore, in order to better gauge the risk of developing complications, the authors conclude that AChR antibody titers should be considered when treating patients with AChR antibody-positive OMG. Patients taking ethambutol should be regularly screened for
optic neuropathy
The medical records of 5394 patients diagnosed
with tuberculosis infection and treated with ethambutol were reviewed to identify
the rate of ethambutol-induced optic neuropathy (EON) and factors affecting
post-EON visual recovery. Twenty- three patients (0.43%) were diagnosed with EON.
A multivariate analysis found that good initial visual acuity (better than
20/200) was the only significant
factor associated with good visual recovery. It is recommended that patients
taking ethambutol have regular ophthalmic screenings to detect EON early; if
EON is diagnosed, ethambutol should be immediately discontinued. Clinical Ophthalmology,

Most open globe injuries occur in men, particularly while
they're at work
A retrospective chart review of 371 consecutive
patients with open globe injury (OGI) was conducted to determine clinical characteristics
and factors associated with no vision outcome (e.g., no light perception, enucleation,
and evisceration) following OGI. Ninety percent of patients were male and 69%
had workplace injuries; the most frequent cause of OGI was a high-velocity metallic
foreign body. Factors associated with no vision outcome were low presenting vision,
globe perforation, larger wound, presence of relative afferent pupillary defect,
retinal detachment, and vitreous hemorrhage. Clinical Ophthalmology,

Fewer failures seen when phacoemulsification is added to trabeculectomy
A retrospective review focused on 121 phakic
patients (146 eyes) with primary glaucoma who underwent either mitomycin C-augmented
trabeculectomy with phacoemulsification (Phaco + Trab) or trabeculectomy alone
(Trab) and completed 2 years of follow-up. The Phaco + Trab group had a
significantly lower failure rate than the Trab group, but differences in postoperative
IOP, percentage of IOP reduction, and number of glaucoma medications were not
significant between the groups. The impact of adding phacoemulsification to
trabeculectomy was found to be similar in eyes with open-angle glaucoma and
