Different Primary Cancers Are Noted for Intraocular and Ocular Adnexal Metastases

 

A retrospective study focused on the clinical characteristics and treatment outcomes of

intraocular metastasis (IM) and ocular adnexal metastasis (OAM). Forty-two patients with either IM or OAM seen at a tertiary center in Thailand were recruited for the study. In the IM group, the lungs were the most common site of the primary tumors (79%), while in the OAM group 33% of primary tumors originated in the breasts. The 1-, 3-, and 5-year overall survival rates were 42%, 18%, and 7%, respectively. Scientific Reports, July 2024

Removing Pterygia Before Cataract Surgery May Reduce IOL Power Calculation Errors

 

A prospective cohort study assessed the impact of pterygium excision on IOL power

calculation and corneal astigmatism in 30 eyes with primary pterygium. IOL power calculation and keratometry were performed prior to surgery and at 1, 3, 6, and 12 months after surgery. At the 12-month postoperative visit, calculated IOL power values, using all formulas, were lower than preoperative IOL values; however, significant changes in calculated IOL power values were only seen between the preoperative and 6-month postoperative visits. The margin of pterygium-induced IOL power calculation error was significantly correlated with greater increases in pterygium horizontal length and area.

Therefore, the authors recommend performing pterygium excision before cataract surgery to minimize postoperative refractive errors. American Journal of Ophthalmology, January 2025

Health Utility Values Can Reflect Visual Function in Patients with Common Eye Diseases

 

To determine how health utility values correlate with visual acuity (VA), investigators recruited participants from outpatient ophthalmology clinics throughout Thailand to fill out several health-related questionnaires, including Time Trade-Off (TTO), the Euro QoL 5-Dimensions 5-Levels (EQ-5D-5L), and the Health Utility Index 3 (HUI3). Responses were received from 309 patients with diabetic retinopathy, wet AMD, or cataract. Overall mean utility values from the TTO, EQ-5D-5L, and HUI3 were 0.84, 0.70, and 0.68, respectively. Two

multivariable mixed-effect models found statistically significant decreases in health

utility with worsening VA, but no significant differences in health utility values among

100% Autologous Platelet-Rich Plasma Shows Noninferiority to 100%Autologous Serum for DED

 

A 4-week, randomized, double-masked clinical trial evaluated 100% undiluted autologous platelet-rich plasma (APRP) and 100% autologous serum (AS) for the treatment of moderate-to-severe dry eye disease (DED). Ninety-six patients with DED who had an Ocular Surface Disease Index (OSDI) score of 223 and/or an Oxford staining grade of 22 were included in the

study. At week 4, there was no significant difference in OSDI score decreases between the APRP and AS groups (9.73 and 8.31, respectively). The estimated OSDI score upper limit was less than the prespecified margin of 4.18 points, indicating noninferiority of 100% APRP. BMJ Open Ophthalmology, October 2024

One Year After Infantile Esotropia Surgery, Favorable Outcomes Are Seen in Many Patients

 

This study reviewed the outcomes of surgery for infantile esotropia in 77 children (mean age 9.5 years) operated on at a tertiary care hospital between January 2013 and June 2023. At the 1-year follow-up visit, 71.4% of children had achieved alignment success, defined as an ocular deviation of <10 PD. A multivariate analysis found that having a preoperative angle

deviation of <50 PD was a prognostic factor for surgical success. Reoperation was required in 15.6% of patients, most commonly due to residual esotropia. No complications were reported during the follow-up period. Clinical Ophthalmology, September 2024

Topical and Systemic Ganciclovir Show Efficacy Against Cytomegalovirus Anterior Uveitis

 

This retrospective cohort study compared the effectiveness of topical vs systemic antiviral treatments in 44 patients with cytomegalovirus anterior uveitis. Topical treatments included 2% ganciclovir (GCV) eye drops and 0.2% GCV eye gel, and systemic treatments included intravenous GCV and oral valganciclovir. All patients in the systemic group and 87% of patients in the topical group responded to treatment; median response time was significantly slower in the topical group than in the systemic group (63 vs 28 days,

respectively). While recurrence rates were lower in the topical group than in the systemic group (37.0% vs 69.2%, respectively), this finding was not significant. Scientific Reports, July 2024

Certain 3-Dimensional AS-OCT Angle Parameters May Be Able to Detect Angle Closure

 

The diagnostic performance of 3- dimensional (3D) angle parameters from swept-source anterior-segment OCT (AS-OCT) imaging were assessed in a cross-sectional study of 118 phakic patients (59 open-angle and 59 closed-angle). Angle opening distance (AOD), angle recess area, and trabecular-iris space area (TISA) at 250, 500, and 750 pm from scleral spur

were measured in 3609 radial-scan images. At a distance of 250 um, the 3D parameters

were found to improve the diagnostic ability of AS-OCT compared with their respective

horizontal 2D parameters. Three of the parameters--AOD 500 pm estimate, AOD 750 pm estimate, and TISA 750 um average--had good ability to detect angle closure. Journal of Glaucoma, October 2024

Bilateral Medial Rectus Recession Outcomes Are Similar With or Without Faden Operation

 

A double-blind, parallel, randomized controlled trial compared the outcomes of bilateral medial rectus recession (BMRc) and BMRc with Faden operation (BMRF) in 40 children aged 3-15 years with large-angle infantile esotropia (260 D). At 6 months after surgery, the mean difference of angle reduction was similar between the BMRc and BMRF groups. Six-month surgical success rates, defined as an esodeviation of <10 D at near fixation, were also similar

Telemedicine Visits Could Be Helpful for Evaluating Facial Dystonia Complications

 

A diagnostic study compared the reliability and usability of telemedicine vs in-person

neuro-ophthalmologist consultations for evaluating facial dystonia and treatment

complications. Forty-three patients with either blepharospasm or hemifacial spasm

each had 2 recorded in-hospital telemedicine visits, along with in-person

visits at baseline and 4-6 weeks later. Neuro-ophthalmologists who performed the

in-person visits re-evaluated the telemedicine videos 8 weeks after they were

recorded. There was substantial agreement between the in-person and telemedicine

visits in detecting lagophthalmos and moderate agreement in detecting spasm.

Adding patient-reported symptoms to the signs of facial dystonia increased both the

sensitivity and negative predictive value in detecting drooping lips and lagophthalmos.
