endophthalmitis often causes poor visual outcomes
Investigators performed a retrospective cohort study of 61 consecutive patients with endogenous endophthalmitis (EE) (74 affected eyes) to determine the characteristics, primary sources, risk factors, and visual outcomes of EE. Five percent of all patients with endophthalmitis were found to have EE. The most common causative agents were Gram-positive bacteria (69%), including Staphylococcus aureus and Streptococcus suls. The majority of the patients had poor visual outcomes, but bilateral infection (rather than unilateral infection) and an initial visual acuity of hand movement or better were associated with better outcomes at the 12-month follow-up visit. Direct urgent CCFs can be predicted with a clinical risk
score
A clinical risk score was developed to predict the likelihood of rapid progression of carotid cavernous sinus fistulas (CCFs), using clinical characteristics of 60 patients with direct and dural CCFs. Multivariate analyses found that age, trauma, orbital bruit, underlying diseases, and logMAR visual acuity were the most significant clinical predictors of risk of direct CCFs, and regression coefficients of these predictors were included in the risk scores. The derived total risk score was accurate in predicting urgent direct CCFs correctly, with an area-under-the-receiving-curve of 97.8%. Using this clinical risk score may help clinicians detect direct CCFs, allowing for timely referral for prompt angiography and appropriate treatment. Clinical Ophthalmology, July 2023

Signs of diabetic neuropathy are seen in OCT angiography Images
To explore the potential use of OCT angiography
(OCTA) to detect diabetic neuropathy (DN), a cross-sectional observational
study stratified 186 eyes into 3 groups according to 24-hour urine albumin
levels: no DN, early DN, and late DN. Retinal and vascular measurements were
taken from OCTA images to determine their association with DN stages.
Vascular density values of all retinal layers were found to be
significantly lower in eyes with DN (early or late) than in eyes with no DN. Mean
fractal dimension, intercapillary area, foveal avascular zone area, central
retinal thickness, and subfoveal choroidal thickness were not
statistically different among the 3 groups. The authors conclude that
vascular density measured by OCTA may be a useful biomarker for DN;
further studies in larger populations are needed to establish a cutoff value

Individual participant data sharing in ophthalmology
journal articles is not common
In response to the 2017 International Council of Journal Medical Editors (ICJME) guideline regarding implementation of de-identified individual participant data (IPD) sharing in clinical trial articles, investigators studied the rate of de-identified IPD sharing in clinical trial articles published by the top 10 ophthalmology journals in 2021. One hundred seventy-five articles were identified; 86 included a statement about their IPD sharing policies. However, only 42 of these 86 articles, less than half, were willing to share IPD, with just 5 using online repositories for sharing data. These numbers indicate a lower-than-expected adherence to ICJME IPD-sharing policies in ophthalmology research despite these policies having been recommended in 2017, several years before article publication. Thrombosis is a rare but serious complication of orbital
varices
A rare case of unilateral orbital varix thrombosis was seen in a 42-year-old woman with a history of bilateral intermittent painless proptosis who presented with right eye proptosis. Computed tomography scans of the orbits revealed both a right orbital varix thrombosis and a left orbital varix. The thrombosis was excised surgically and an intralesional bleomycin injection was given in the left orbital varix. Good clinical improvement was seen at the 6-month follow-up visit. Many outward signs of pseudoexfoliation cannot be confirmed
with microscopy
Using light microscopy and transmission electron microscopy, researchers investigated features of the anterior lens capsule in 102 patients with preclinical pseudoexfoliation syndrome, defined as the presence of pigmented spoke-wheel deposition, midperiphery cleft/lacunae, faint central disc present within the photopic pupil, and a white-spoke pattern noted at the midperiphery. Light microscopic analysis did not detect any definite pseudoexfoliation materials in the excised anterior lens capsules, while transmission electron microscopic analysis found pseudoexfoliation material precursors in only 1 specimen. True exfoliation syndrome was associated with the presence of midperiphery cleft/lacunae and faint central disc. PLOS One, March 2023

The Epstein-Barr virus
can induce glaucoma after repeat penetrating keratoplasties
In this case study, a 78-year-old woman presented with a markedly edematous corneal graft, dense pigmented keratic precipitates, and ocular hypertension of 55 mm Hg in the left eye the day after a third penetrating keratoplasty. An aqueous tap for polymerase chain reaction analysis was positive for Epstein-Barr virus (EBV) DNA. The patient was diagnosed with both anterior uveitis-induced malignant glaucoma and EBV-related endotheliitis, which were treated with topical 2% ganciclovir eye drops and oral valacyclovir. Even in HIV-negative
patients, cytomegalovirus retinitis has a high recurrence rate
This retrospective cohort study investigated the incidence of and predictive factors for recurrent cytomegalovirus retinitis (CMVR) in HIV-negative patients with CMVR. One-third of the 44 eyes studied developed CMVR recurrence after discontinuation of anti-CMV therapy (average discontinuation time: 2.6 months). Patients who developed CMVR recurrence tended to have less intraocular inflammation at presentation and less improvement in visual acuity at 6 months than patients without recurrence. Multivariable analyses showed that predictive factors for CMVR recurrence were large retinitis size, absence of vitreous haze, and neutropenia at presentation. Receiving systemic immunosuppression in the 3 months before presentation was significantly associated with a lower risk of CMVR recurrence. Ocular Immunology and Inflammation, February 2023

A 25-gauge needle can
be used to salvage the dislocated haptic of a flanged IOL
This report describes the development of a simple technique that was used to salvage the dislocated haptic of a flanged IOL during a retinal detachment surgery. A 25-gauge regular-wall needle was first prepared and threaded through the dislocated flanged haptic, and low-temperature cautery was used to heat and enlarge the flanged haptic. Donor cells can remain for months after simple limbal
epithelial transplantation
This noncomparative cohort study used genetic analysis to assess long-term donor cell survival after successful allogenic simple or cultivated limbal epithelial transplantations (allo-SLET/allo-CLET) in 12 patients with bilateral limbal stem cell deficiency. DNA analysis was performed during the postoperative period. Donor cells were detected in 4 of the 8 patients who underwent allo-SLET and none of the patients who underwent allo-CLET; in one patient, donor cells were still detected 30 months after surgery.
